A 72-hour fast (also called a 3-day fast or 3-day water fast) produces measurable metabolic changes — but it carries real risks and is contraindicated in a significant number of people. During hours 0–24, your body burns through stored carbohydrate (glycogen). Between hours 24–48, it transitions fully to fat-burning and ketone production, with insulin dropping sharply. Search interest for "72 hour fast" has roughly tripled in Canada over the last 24 months, alongside a flood of YouTube and TikTok content describing it as a metabolic reset, an autophagy switch, or a longevity hack. Most of those videos are made by people without clinical training, working from a small handful of studies they've half-read.
The actual research is more interesting than the influencer take and more specific than "fasting is good for you." A 72-hour fast does shift the body in measurable ways — hormonally, metabolically, and at the cellular level. It's also genuinely risky in some populations and badly suited to others.

The metabolic story is more useful than calorie math.
- Hours 0–12. Normal post-meal state. Insulin elevated, body using carbohydrate as primary fuel. Liver glycogen reserves: roughly 100 g.
- Hours 12–24. Insulin starts dropping. Glycogen depletes. Body begins shifting toward fat oxidation. This is the standard intermittent fasting zone — the range a daily schedule like 16:8 or 5:2 lives in.
- Hours 24–48. Glycogen tapped out. Body fully transitioned to fat-burning. Ketone production ramps up. Insulin near baseline. Hunger paradoxically eases for many people in this window.

The 48–72 hour window is where most of what makes a "72-hour fast" different from a 24-hour fast actually happens. If you stop at 36 hours, you've done a long fast — but you haven't done that fast.
Is a 3-day fast the same as a 72-hour fast?
Same fast, two names. About half the people looking for this information search "3-day fast" rather than "72-hour fast," and many of them specifically mean a 3-day water fast. Two versions get blurred together online that shouldn't be:
- A "3-day fast" done with broth, juice, or a fasting-mimicking meal kit is a very-low-calorie diet, not a fast. Some of the metabolic shifts still happen, but slower and less completely.
- Three separate 24-hour fasts spread across a week are a different thing entirely. The growth-hormone and autophagy changes described above build across continuous fasted hours. Reset the clock each day and you never leave the 12–24-hour zone.

The real risks
For healthy adults, the research on supervised multi-day fasts is reassuring. For everyone else, it's a mixed bag.
Common, low-severity:
- Dehydration — the leading reason 72-hour attempts fail
- Headaches, especially days 1 and 2
- Dizziness on standing (low blood pressure)
- Difficulty concentrating
- Cold extremities
- Trouble sleeping on night 2 or 3
Less common, higher-severity:
- Electrolyte imbalance (sodium, potassium, magnesium) — can trigger heart-rhythm issues
- Hypoglycemia — especially if you take any blood-sugar-lowering medication
- Gout flare — uric acid rises during extended fasts
- Refeeding syndrome on the way out — uncommon at 72 hours, but possible
Serious, fast-stopping:
- Heart palpitations or irregular heartbeat
- Confusion, fainting, or severe weakness
- Chest pain
- Vomiting
If any of those last four appear, end the fast and contact a clinician. Don't push through.
Who should never attempt a 72-hour fast
There is a meaningful list of exclusions where the risk-benefit math tips clearly against trying this:
- Pregnant or breastfeeding
- Under 18, or over 70 without clinical supervision
- Type 1 diabetes, or type 2 diabetes managed with insulin or sulfonylureas
- History of an eating disorder
- Underweight (BMI under 18.5)
- Advanced kidney or liver disease
- Gout
- Currently taking blood-pressure, heart-rhythm, or seizure medications without clinician oversight
- Several antidepressants (lithium especially)
If you're in any of these groups, the answer is not "do it carefully" — the answer is "don't, or only under direct medical supervision."
How to actually do it (if you've decided to)
For healthy adults without contraindications, the structure that works for most members:
- Pick the right week. Avoid weeks with heavy work travel, intense workouts, social commitments, or major sleep disruption.
- Eat clean for 48 hours before. A few days of stable, lower-carbohydrate eating before the fast makes the first 24 hours far easier.
- Plan your hydration. Target 2.5–3 L of water daily across the 72 hours. Add electrolytes — a pinch of salt in water, or 500 mg sodium + 250 mg potassium + 100 mg magnesium per day in an unsweetened mix.
- Skip exercise. Walking is fine. Lifting weights, HIIT, or long runs are not — your glycogen is gone and your cardiovascular reserve is lower than usual.
- Sleep and rest. This is genuinely a rest period. Night 2 sleep is often disrupted; that's normal.
- Break the fast carefully. Small protein-and-fat meal first. Wait 90 minutes. Then resume normal eating.
A useful rule of thumb: the harder this is going, the more likely you should stop. Extended fasting should feel surprisingly manageable for someone in the right shape to try it. If it feels punishing, that's a signal — not a badge.
