How to Measure Your Waist Correctly — Canada
Your waist circumference is one of the most informative health measurements you can take — and most Canadians have never been taught how to take it correctly. Why your waist measurement matters more than the scale
Weight is easy to measure but remarkably limited as a health signal. It cannot distinguish muscle from fat, it cannot tell you where fat is stored, and two people who weigh exactly the same can carry very different metabolic risk depending on their body composition and fat distribution.
Waist circumference solves for the single most important blind spot: it directly reflects the amount of central fat you are carrying, and central fat is where metabolic risk lives. The fat stored deep in the abdomen, around the liver, pancreas, and intestines (what clinicians call visceral fat), is biologically active in ways that subcutaneous fat, the softer fat just under the skin, is not. It releases inflammatory signals and free fatty acids directly into the blood supply feeding the liver, which is why it has an outsized effect on insulin sensitivity, blood pressure, and lipid profiles.
The scale cannot see any of this. A waist measurement can.
This is why Health Canada, the Canadian Obesity CPG, and most major cardiology guidelines use waist circumference, not weight and not BMI alone, as a core clinical screening measure. The correct technique
A waist measurement taken incorrectly is worse than inaccurate: it can give you false reassurance or unnecessary alarm. The technique is simple, but precision on a few details matters.
What you need
A soft, flexible tape measure (a fabric or fiberglass sewing tape, not a metal builder's tape). You do not need a special medical instrument — the same tape used for tailoring works perfectly.
Step 1: Find the correct landmark
The correct measurement site is the midpoint between the top of your hip bone (the iliac crest) and the bottom of your lowest rib. Run your fingers down your side until you feel the flare of your hip bone, then feel up to find the lowest rib — the measurement goes halfway between them.
This is more reliable than measuring at the belly button, which many guides still recommend. For most people with a typical fat distribution, the navel lands close to the correct anatomical point. But if weight has shifted or the belly has changed shape over time, the navel can drift from its original position while the iliac-crest midpoint stays consistent. Using the anatomical landmark keeps your measurements comparable over time.
Step 2: Position the tape
Wrap the tape horizontally around your waist at the landmark you have identified. Stand straight, feet shoulder-width apart, arms relaxed at your sides. Check in a mirror that the tape is level all the way around. It should form a horizontal circle, not dip at the front or ride up at the back.
Step 3: Get the tension right
The tape should be snug but not compressing. You want it in contact with the skin all the way around without indenting it. If you can slide a finger under the tape, it is probably too loose. If the tape is visibly pressing into the skin, it is too tight. Both errors skew the reading.
Step 4: Breathe and read
Take a normal breath in, breathe out gently, and read the tape at the end of that exhale — before your next breath in. This is when your waist is at its natural resting circumference. Record the number to the nearest millimetre if possible, or to the nearest half-centimetre at minimum.
Do not suck in. Do not inflate your chest and hold your breath. Just breathe out normally and read.
Consistency for tracking
If you are measuring to track change over time, after starting a new lifestyle program for example, take the measurement at the same time of day each session (morning before breakfast is conventional) and in the same clothing or lack thereof. Day-to-day variation of one to two centimetres is normal; trends over weeks and months are what matter.
Canadian waist circumference thresholds
The thresholds most commonly used in Canadian clinical practice come from Health Canada and the National Heart, Lung, and Blood Institute (NHLBI), and they are the same values referenced in the Canadian Obesity CPG and most provincial guidelines.

In clinical practice, the cutoffs most often cited as the action threshold are 88 cm (35 inches) for women and 102 cm (40 inches) for men. The lower "elevated risk" values — 80 cm for women and 94 cm for men — reflect a gradation of risk that is common in European and WHO guidelines and increasingly acknowledged in Canadian practice.
Ethnicity matters here. South Asian, East Asian, Filipino, Japanese, Chinese, and some other populations carry elevated cardiovascular and metabolic risk at lower waist measurements than the NHLBI thresholds suggest. If this applies to you, the clinically relevant cutoffs may be closer to 80 cm (31.5 in) for women and 90 cm (35.5 in) for men. Your family physician or a clinician familiar with your background is the right person to interpret your specific reading.
These thresholds are also sex-specific, not one-size-fits-all, because fat distribution differs meaningfully between men and women. Women naturally carry more fat, and the clinical significance of waist circumference differs accordingly — which is why the numbers are not the same.

Waist-to-height ratio: a better predictor than waist alone
Waist circumference thresholds have a real limitation: they do not account for height. A waist of 90 cm carries very different implications depending on whether the person measuring it is 155 cm tall or 190 cm tall. A number that signals elevated risk in a shorter person may be unremarkable in a taller one, and vice versa.
Waist-to-height ratio (WHtR) corrects for this. You divide your waist circumference by your height, both measured in the same units:
WHtR = waist ÷ height
The widely used target is simple: keep your waist under half your height — a WHtR below 0.5. A reading of 0.5 or above suggests central fat accumulation worth discussing with a clinician. Values above 0.6 are associated with substantially elevated cardiometabolic risk.
Because it scales to body size, WHtR predicts metabolic syndrome, insulin resistance, cardiovascular events, and type 2 diabetes risk more consistently than waist alone, and in most studies more consistently than the BMI calculator. It is not a diagnosis, but as a first-pass screen it works well across a wider range of body types and ethnic backgrounds than fixed-cutoff waist measurements. Common mistakes that skew your measurement
Even with a tape measure and good intentions, a few common errors can push your reading significantly off:
Measuring at the belly button instead of the anatomical midpoint. For most people these are close — but not always. Use the iliac-crest-to-lowest-rib midpoint as your landmark.
Letting the tape slip lower at the back. The tape should circle the body horizontally. If it dips at the back, you are measuring a different location than you think, and typically a smaller one, which gives false reassurance.
Measuring after a large meal or later in the day. Abdominal volume increases after eating and tends to be lowest in the morning. For tracking purposes, stick to a consistent time.
Holding your breath in or sucking in. Both significantly change the reading. Breathe normally and measure at the end of a relaxed exhale.
Pulling the tape too tight or leaving it too loose. Both errors skew the result in opposite directions. Snug-but-not-compressing is the correct standard.
Wearing bulky clothing. Take the measurement against bare skin or a single thin layer. Clothing — especially a waistband or belt — changes where the tape sits and adds girth to the reading.
Measuring once and assuming precision. Take two readings in quick succession and average them. A one-centimetre difference between readings is common and normal; the average gives a more stable estimate.
What your number means — and what to do with it
A waist measurement above threshold does not mean something is wrong beyond repair. It means you have a signal worth paying attention to. Context is everything:
- Below both the elevated-risk and high-risk threshold for your sex. This is reassuring, but waist circumference is one measure among several. If you have other metabolic risk factors — family history, prediabetes, high blood pressure, high triglycerides — a conversation with your clinician still makes sense.
- Between the elevated-risk and high-risk thresholds. This is a yellow-flag zone. Lifestyle adjustments — dietary quality, physical activity, sleep, stress — are the evidence-supported first steps, and they are often effective. Tracking change over time with repeat measurements is useful.
- Above the high-risk threshold. This warrants a clinician visit. A single elevated measurement is not a diagnosis, but above the high-risk threshold for your sex it sits in the range where metabolic syndrome, insulin resistance, and cardiometabolic complications are meaningfully more likely. Blood work can fill in the picture.
Visceral fat, the fat your waist measurement is standing in for, is among the most responsive to sustained lifestyle change. The same fat that is metabolically most dangerous also tends to mobilize well when diet, activity, and sleep improve. You are not locked into where you are.
When to see a clinician
A waist measurement above the high-risk threshold for your sex is one clear prompt for a clinical conversation. But several other scenarios are also worth bringing to a clinician:
- Your waist is within normal range but trending upward over months, since central fat often accumulates gradually before crossing a threshold.
- You have a family history of type 2 diabetes, cardiovascular disease, or metabolic syndrome, which shifts your personal risk profile even at a lower waist measurement.
- You have PCOS or another condition linked to insulin resistance, where central fat has particular clinical significance.
- You have already made sustained lifestyle changes and your waist is not responding — a clinician can look for compounding factors and assess whether a structured program, or prescription options a Canadian clinician may consider when clinically appropriate, are part of the next step.
