Tirzepatide — Product Information
Tirzepatide is a synthetic peptide and a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist. It is administered as a once-weekly subcutaneous injection.
How Tirzepatide Works
Tirzepatide activates both GIP and GLP-1 receptors simultaneously. This dual-receptor mechanism:
- Enhances insulin secretion in a glucose-dependent manner
- Suppresses glucagon secretion
- Slows gastric emptying
- Reduces appetite and caloric intake
Tirzepatide differs from single-agonist GLP-1 medications (such as semaglutide) by also activating the GIP receptor.
Tirzepatide Products in Canada — Approved Indications
Mounjaro® (Health Canada–approved 2022)
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus:
- As monotherapy when metformin is considered inappropriate
- In combination with other medicinal products for the treatment of type 2 diabetes
Zepbound™ (Health Canada–approved 2025)
Indicated for chronic weight management in adults as an adjunct to a reduced-calorie diet and increased physical activity, in adults with:
- A body mass index (BMI) of ≥30 kg/m² (obesity), or
- A BMI of ≥27 kg/m² with at least one weight-related comorbidity (hypertension, type 2 diabetes, dyslipidemia, obstructive sleep apnea)
Clinical Trial Programs
SURPASS (Mounjaro, type 2 diabetes): Across SURPASS-1 through SURPASS-5 (n=>13,000 adults), tirzepatide 15 mg reduced HbA1c by 2.0–2.4% on average.
SURMOUNT (Zepbound, chronic weight management): In SURMOUNT-1 (n=2,539), tirzepatide 15 mg produced an average 20.9% body weight reduction over 72 weeks vs 3.1% on placebo.

Available Dose Strengths
Both Mounjaro and Zepbound are available in the same dose strengths: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg per weekly injection. Dosing is titrated upward over time based on clinical response and tolerability.
Common Side Effects
Most common (≥5% of patients): nausea, diarrhea, vomiting, constipation, abdominal pain, decreased appetite, injection site reactions. Most occur during dose titration and typically decrease over time.
Important Safety Information
Tirzepatide is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Tirzepatide should not be used in patients with type 1 diabetes mellitus. Discuss your full medical history with your healthcare provider before starting any tirzepatide product.
