HOMA-IR is one of the most widely used quick estimates of insulin sensitivity, and one that is easy to misread without context. The calculator below turns two fasting blood-work values — your fasting glucose and your fasting insulin — into a HOMA-IR estimate in seconds, accepting glucose in either mmol/L (the Canadian standard) or mg/dL. Just as importantly, it is clear about what the number is not: a single estimate, not a diagnosis, and one that always belongs in a conversation with your clinician.
HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) estimates insulin sensitivity from fasting blood work using the formula (fasting glucose in mmol/L multiplied by fasting insulin in micro-units per mL) divided by 22.5. In many general adult populations a value under about 2 is cited as consistent with good insulin sensitivity, but reference ranges vary widely between labs, assays, and populations. HOMA-IR is a research and screening estimate, not a diagnostic test: insulin assays are not standardized, a single value can be noisy, and it reflects only fasting conditions. Never use it to diagnose a condition or change treatment — review your actual fasting blood work with a clinician.
How HOMA-IR is calculated
The formula is short and has been used in research for decades:
HOMA-IR = (fasting glucose × fasting insulin) ÷ 22.5
Here fasting glucose is in mmol/L and fasting insulin is in micro-units per millilitre (µU/mL). If your lab reports glucose in mg/dL, the value is first divided by about 18 to convert it to mmol/L. So someone with a fasting glucose of 5.5 mmol/L and a fasting insulin of 10 µU/mL has a HOMA-IR of (5.5 × 10) ÷ 22.5, which works out to roughly 2.44. The calculator above does the unit conversion and the arithmetic for you and rounds to two decimal places.
The constant 22.5 is a normalizing factor built into the original model. The maths is deliberately simple, which is part of why HOMA-IR became so popular as a screening estimate — and also part of why it should be read with care rather than precision.
How to interpret your estimate
It is tempting to want a clean threshold, but HOMA-IR does not offer one universal cut-off. In many general adult populations a value under about 2 is described as consistent with good insulin sensitivity, with higher values used as a prompt to look more closely. The bands shown by the calculator reflect that rough, commonly cited context.
The crucial caveat is that reference ranges vary substantially. Different laboratories use different insulin assays, and those assays are not standardized against one another, so the same blood sample can produce different insulin values — and therefore different HOMA-IR figures — depending on where it is run. Populations also differ. This is exactly why the number from any online tool is context, not a verdict, and why your own lab's reference range and your clinician's reading carry far more weight.
Why HOMA-IR is an estimate, not a diagnosis
We want to be especially clear about this, because HOMA-IR is so often over-interpreted. The estimate has genuine limitations for any single person:
- Assays are not standardized. Fasting insulin results differ between labs, which directly shifts the HOMA-IR value.
- One reading is noisy. A single fasting measurement can be affected by recent sleep, stress, illness, and natural day-to-day variation.
- It reflects only the fasting state. HOMA-IR captures a snapshot at rest; it does not describe how your body handles glucose after eating.
- It is one input, not the whole picture. Clinicians weigh it alongside history, symptoms, other markers, and risk factors — never in isolation.
None of this makes HOMA-IR useless. It makes it a low-cost screening estimate that can help a clinician decide whether a closer look is worthwhile. Used as a label or a self-diagnosis, it can mislead and cause needless worry — which is why this tool is framed as a starting point for a conversation, not an answer.
